Registered dietitians ensure proper recovery and wellness through nutrition counseling and education.
General skilled nursing
Experienced, registered home care nurses work with your Prisma Health doctor or surgeon to provide medical and post-surgical care, medication education, disease management, intravenous (IV) fluids, and durable medical equipment (beds, walkers, commodes/toilets).
Home health aides
Certified aides provide personal care services, such as bathing, in the privacy and comfort of your home.
Licensed, experienced occupational therapists retrain the patient’s arms and hands to increase independence and the ability to manage daily living activities.
Our licensed, experienced physical therapists help strengthen the patient and improve walking and balance, while teaching caregivers how to assist.
Licensed, experienced social workers assess the emotional and social needs of the patient and family, assist with coping skills, and identify community resources.
Licensed, experienced speech therapists maximize the patient's ability to communicate and help resolve swallowing disorders.
30-day recovery care
In the Upstate area, you can safely receive recovery care and treatment at home for 30 days. Your Prisma Health doctor works directly with your assigned registered nurse, who cares for you at your home twice a day for 1–3 hours each visit. Any equipment, such as an IV, will be set up for you. We will also lend you a computer tablet, at no charge, that lets you see and talk to your doctor from home.

- Congestive heart failure
Specialty in-home services
Specially trained nurses provide insulin administration and instruction, glucose monitoring, diabetic diet, wound care and lower extremity care.
Infusion care for patients receiving chemotherapy, antibiotics, hydration and nutritional therapies.
Neonatal and pediatric services
Pediatric nurses and physical, occupational and speech therapists provide care for newborns and children with a broad range of diagnoses.
Planned Approach to Home Care (PATH)
Care for patients with a potentially life-limiting illness who choose to continue receiving their treatments at home.
Women's health and maternity
Experienced OB/GYN nurse care for women with pregnancy-related conditions or other post-surgical needs.
Consultation and treatment for ostomy care and wound healing.
Other nursing services
The Transitional Care Program, available in the Upstate, bridges the gap between the hospital stay and the first follow-up visit. A team of doctors, nurses, pharmacist, dietitian and social worker meet with the patient once a week for four weeks to monitor recovery and help patients learn to manage a chronic illness.
A long-term option for adults who can live on their own but need some help with daily tasks such as cooking, cleaning, taking medications, bathing or dressing. Includes nursing care, social programs and access to medical care.
Palliative care – inpatient, outpatient or home-based
A special team of compassionate doctors, nurses and other experts work together with a patient’s other doctors to help manage the stress and symptoms of serious illness, at any stage of the condition.
Skilled nursing home
Our 24/7 facilities provide a full range of short- and long-term care and services for patients who cannot perform daily living tasks safely on their own. Includes nursing care, therapy, social programs and access to medical care.
